Responsibilities
We are seeking a compassionate and experienced Full-Time ARNP to join our dedicated team at Franciscan Hospice and Palliative Care. This job is responsible for working with the Hospice and Palliative Medicine physician(s) team in providing specialized home hospice and palliative care services that provide for quality, efficient and effective patient care in accordance with Franciscan Health System (FHS) standards/guidelines, applicable statutory requirements and accepted clinical practice for the nurse practitioner profession. An incumbent is engaged in consulting for patients as requested by the attending physician, following established protocols as determined by Palliative Medicine standards. Work includes providing direct patient care by examining, diagnosing and analyzing total patient health condition and developing short- and long-term goals and treatment plans in collaboration with the attending physician and/or established protocols/standards. Nurse practitioners are familiar with the patients throughout the continuum of care, and they tailor care and education to meet each patient's needs.
This opening is for the Kitsap County Area and involves visits at patient homes and facilities, as well as remote work. The schedule is Monday-Friday.
Hospice Responsibilities Include:
- Conduct comprehensive medical evaluations and assessments for hospice patients including face-to-face encounters to establish continued hospice eligibility
- Develop and implement individualized treatment plans for managing pain and other symptoms, ensuring patients' comfort and quality of life
- Work in collaboration with the hospice team physician and interdisciplinary care team
Palliative Care Outreach Responsibilities Include:
- Consulting on patients with serious, life-limiting illnesses in their homes / facilities and coordinating care with their primary teams (e.g. PCP, oncology, neurology, etc.)
- Give recommendations for managing pain and other symptoms
- Assist patients and families with complex decisions about medical treatments
- Work with patients and families to develop advance care planning documents
Qualifications
Requires licensure as an Advanced Registered Nurse Practitioner (ARNP), with prescriptive authority. An incumbent exercises a high degree of independent judgment and applies advanced skills obtained through academic and clinical preparation. Unusual problems or proposed deviations from guidelines, practices or precedents are discussed with a physician before being initiated.
- ARNP degree
- Minimum of 2 years of experience in hospice or palliative care, or a related field working with terminally ill patients
- Washington State Medical License and DEA in good standing with no restrictions
- Excellent communication and interpersonal skills
- Passion for providing compassionate end-of-life care
