Mercy Medical Group (MMG), a multi-specialty group with over 500 healthcare providers across the Sacramento region, is seeking a dedicated Advanced Practice Provider (APP) to join our dynamic and expanding Geriatrics division.
About the Role:
As an integral part of our comprehensive Geriatrics team, the APP will provide personalized chronic disease management for complex elderly patients in their homes, assisted living facilities, and board and care settings. You’ll play a key role in reducing unnecessary ER visits and hospitalizations by supporting primary care providers and delivering high-quality, compassionate care.
You'll collaborate with an award-winning interdisciplinary team of geriatricians, social workers, pharmacists, case managers, and fellow APPs to ensure the best outcomes for our aging population. Community Description:

Responsibilities
This position would involve visiting complex patients in their homes, assisted living facilities and board and care facilities in support of PCP’s managing multiple medical conditions, to prevent unnecessary ER visits or hospital admissions. The successful candidate will work with a complete interdisciplinary team including Geriatricians, social workers, pharmacy and case management.
Qualifications
- A valid and unrestricted license to practice in the State of California
- Nationally recognized certification (as determined by Medicare)
- Completion of state mandated educational courses
