The posted compensation range of $68.00 - $78.00 /hour is a reasonable estimate that extends from the lowest to the highest pay CommonSpirit in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. CommonSpirit may ultimately pay more or less than the posted range as permitted by law.
As a Family Medicine Nurse Practitioner or Physician Assistant with Healthy Living Primary Care, you will play a crucial role in delivering comprehensive, patient-centered primary care. You will join an integrated healthcare team dedicated to preventative medicine, holistic patient care, and leveraging advanced technology to enhance both physical and mental well-being for our patients in Folsom or Fair Oaks. This is an exciting opportunity for an APRN or PA-C to make a significant impact within a highly-rated primary care network known for exceptional patient satisfaction and physician autonomy, located in the Greater Sacramento Area.
Every day you will provide primary and preventative care to a diverse patient panel, including adults and pediatric patients. Your responsibilities will include managing stable chronic medical conditions, conducting Medicare annual wellness exams, and performing various procedures. You will also conduct lab and imaging follow-up appointments, with ample time allocated for each patient encounter (30 minutes), ensuring quality of care over volume. Healthy Living Primary Care emphasizes a collaborative environment where you can thrive, supported by tenured staff and access to state-of-the-art facilities offering behavioral health, weight management, and chronic/remote patient management services.
To be successful in this role, you will be a board-certified Nurse Practitioner (NP) or Physician Assistant (PA) with a passion for family medicine and a holistic approach to patient care. You should possess strong clinical skills in managing common primary care conditions and be adept at leveraging technology to enhance patient outcomes. A commitment to work-life balance, a collaborative spirit, and a desire to contribute to a highly-regarded healthcare team are essential. Experience with both adult and pediatric populations, as well as a focus on preventative strategies, will be highly valued in this outpatient primary care setting.

Job Requirements
Required
- Completion of accredited training program(s)
- Active California NP or PA license (or ability to obtain)
- Board certified
- 2-5 years prior APP experience in family medicine
- Exceptional communication and interpersonal skills
- Ability to work effectively in a team environment
- Commitment to providing compassionate and patient-centered care
- Willingness to learn and grow
Preferred
- Ability to speak Spanish and/or Russian
- Prior APP experience seeing pediatric patients
