The Marian Regional Medical Center Family Medicine Residency Program is a community-based training experience designed to prepare residents for full-spectrum family medicine in diverse inpatient, outpatient and underserved settings. Located in Santa Maria, California, the program focuses on developing highly skilled, compassionate, and evidence-based practitioners.
As part of Dignity Health, we are a member of CommonSpirit Health's national network of more than 2,200 care sites across 24 states. That broad perspective supports our local commitment to community-based training, health equity and compassionate care.
Residents train across various settings, including the state-of-the-art hospital and the dedicated Dignity Health Family Medicine Center, serving a diverse patient population, including Hispanic, Caucasian, Pacific Islander and African American ethnicities in Santa Maria and the broader Central Coast.
Upon graduation, residents are equipped to provide excellent, compassionate, evidence-based care in any environment, adapt to the rapidly changing healthcare landscape and navigate complex healthcare systems.

Our residents go beyond traditional practice, immersing themselves in the community to meet patients where they are—from shelters and schools to clinics and emergency departments. We offer comprehensive training in procedures, trauma-informed care, and Osteopathic Manual Medicine, all within our ACGME-Osteopathically Recognized and CommonSpirit Health Medical Safe Haven programs.

The Marian Family Medicine Residency Program is a three-year post-graduate program, ACGME-accredited with Osteopathic Recognition, designed to provide residents with comprehensive training in the full spectrum of family medicine across various inpatient, outpatient, and community settings. The program is approved for six residents per year, totaling 18 residents.
The first year focuses on foundational training, including Family Medicine Inpatient Service, Prenatal and Obstetrics, Inpatient Pediatrics, Outpatient Pediatrics, Cardiology, General Surgery, Point-of-Care Ultrasound (POCUS), Scholarly Activity, Family Medicine Clinic and Elective rotations.
The second year provides expanded clinical responsibility with rotations in Family Medicine Night Float, Emergency Medicine, Musculoskeletal Medicine, Endocrinology, Community Medicine/Underserved, Inpatient Pulmonology and Critical Care, Family Medicine Clinic, Oncology, Urology, Scholarly Activity, ENT, Geriatrics, Dermatology and Elective rotations.
The third year prepares residents for independent practice through rotations in Family Medicine Inpatient Service, Neurology, Psychiatry, Outpatient Pediatrics, Gastroenterology, Family Medicine Clinic, Outpatient Procedures, Scholarly Activity, Cardiology, Rheumatology and extended elective time.
Every Thursday afternoon, residents have protected time dedicated to didactic sessions. The curriculum is structured on an 18-month cycle, covering categories outlined in the ABFM blueprint, along with topics such as practice management, resident/faculty meetings, resident-only meetings and wellness activities.
The PGY-2 year includes an eight-week rotation in Family Medicine Night Float, providing experience with overnight patient care responsibilities. Residents also complete 24-hour call four times per year during both PGY-2 and PGY-3.
Residents participate in Scholarly Activity rotations throughout all three years. The program also embraces a culture of safety and patient-centered medical home principles, focusing on continuously improving team-based care and utilizing Just Culture methodology to enhance patient safety.
Dedicated elective blocks are integrated into each PGY year, offering opportunities for residents to explore specific interests within family medicine. Residents complete 2 weeks of elective training in PGY-1, 4 weeks in PGY-2 and 16 weeks in PGY-3, totaling 6.5 blocks of elective experience.
